Before Your Arrival

We have all been there and we know how embarrassing it can be to have shaky handwriting when filling out paperwork during that first visit. That’s why we provide you with the opportunity to complete all necessary documentation right here, online, before your first appointment.

At our Online Patient Registration page, you can complete the form that will help us understand your needs. This information enables us to give you the best possible healthcare. The information on the form is completely confidential and will be erased from the system as soon as you click on Submit.

When You Arrive for Your First Visit

Now that you have the paperwork part out of the way, we can settle right down to the business of your healthcare. The goal? Soothe your pain and make you feel better. After one or several visits, the doctor corrects muscles and tissues so they can heal completely. Once healed, periodic adjustments will keep them that way.

Payment Options

We understand that everyone has a unique financial situation so we offer a number of options to meet a variety of budgets.

Insurance

We accept most insurance plans as well as auto accident, workers compensation, personal injury and Medicare. Our advice: call our office to find out if your plan covers care in our office.

No Insurance

If you do not have insurance, we will work with you to design a flexible payment plan that works within your budget. I think you will find our excellent care, and the relief you will get, affordable.

Family Plans

Our focus for your entire family is proper health, wellness and disease management. To help you achieve total wellness, we will work with you to create a customized family plan for payment.
